The membership secretary reviews your answers and creates a trial membership record.

Welcome to Academy Swim Team Burnham-on-Sea. Please complete all four parts below and submit to the club's membership secretary. If the member is under 18, provide parent/guardian contact details rather than the young person's. If you have multiple swimmers joining, submit this form once for each swimmer. Questions? Part 1 — Membership Form

Basic details for the swimmer, volunteer, or adult member.

Choose the option that best matches this application. Volunteers (non-swimming club support) are registered as Swim England Club Support.

Volunteers: use your own email. Swimmers under 18: use a parent/guardian email — this is used for website login.

Your own number. For swimmers under 18, a parent/guardian mobile is fine.

Optional. Leave blank if you do not have one yet — we can add it later.

This is a preference only — the club will confirm the final category. Club Train — for training only. Most new swimmers start here, then move to Club Compete when they feel comfortable entering competitions. Club Compete — for swimmers who want to enter galas and competitions (required for meet entries). Not sure yet — leave the decision to the club. Volunteer-only applications are set to Club Support automatically.

For competition categories (Swim England): Female = birth sex female; Male/Open = birth sex male, transgender, non-binary, or any competitor not eligible for female category.

Guardians (under 18)

For under-18 swimmers, guardians are the emergency contacts.

You can include relationship in the name (e.g. Alice Osgood — Mum).

Optional second guardian. You can include relationship (e.g. Sam Osgood — Dad).

Emergency contact

Someone we can call in an emergency. Volunteers: please provide an adult emergency contact. Swimmers under 18: guardians above are usually enough; add another contact here if useful. Adults (18+): use these fields.

Include relationship if helpful (e.g. Jamie Smith — Partner).

Part 2 — Photography Declaration & Sharing of Achievements

Photographs of under-18s are taken and published in line with Swim England's Photography Policy. You can withdraw consent in writing to the Welfare Officer at any time.

For example after winning medals or in group photos.

A useful tool for stroke technique.

We'd love to let your school know about news or achievements.

Part 3 — Medical Information & Declaration

To keep members safe we need to know about their health and have prior consent to act in a medical emergency.

Include medication that may be required in an emergency (e.g. asthma, epilepsy, diabetes, cardiac conditions). Brief care-plan notes here; submit full plans to [email protected].

State if there has been a previous anaphylactic reaction and if an epipen or similar has been prescribed.

Let us know particular ways they learn or engage.

e.g. deafness, eyesight or joint problems. Members with disabilities can sometimes get exemptions or further support.

GP name, surgery name, address and phone number (held for emergency use, e.g. passing to ambulance crew).

Type your first and last name to give permission for the coach, team manager or authorised person to authorise emergency medical or surgical treatment recommended by competent medical authorities where delay seeking personal consent would be contrary to medical interest.

Part 4 — Membership Agreements

As the parent/guardian of an athlete under 18 I give permission for my child to participate in AST club events at Burnham Swim and Sports Academy and other UK mainland venues they are invited to.
